Zenec Home Care & Community Services is a
dedicated provider of high-quality NDIS disability and community support services in Sydney,
committed to empowering individuals to live independently, safely, and with dignity. Our
organisation was founded on the belief that every person deserves compassionate care, respect,
and the opportunity to achieve their goals regardless of the challenges they face. We provide
person-centred, culturally respectful, and professional support tailored to the individual needs of
each participant. At Zenec Home Care & Community Services, we work closely with hospitals, rehabilitation centres, mental health
services, community corrections, and other social service organisations to ensure participants
receive the right support when transitioning into the community.

Supporting Safe Transitions from Hospital and Rehabilitation

We understand that the transition from
hospital or rehabilitation back into the community can be a critical and vulnerable period for many
individuals. Zenec Home Care & Community Services provides reliable support services to ensure participants receive the care and
structure they need after discharge. Our team works collaboratively with hospital discharge
planners, rehabilitation teams, and allied health professionals to deliver coordinated support aligned
with each participant’s NDIS plan.

Mental Health and Psychosocial Disability Support

Zenec Home Care & Community Services has a strong commitment to supporting
individuals living with mental health conditions and psychosocial disabilities. Through our Recovery
Coaching and community participation programs, we help participants rebuild confidence, develop
life skills, and reconnect with their communities.

Justice and Community Reintegration Support

Zenec Home Care & Community Services also provides specialised support for NDIS
participants transitioning from the justice system into the community. We work with community
corrections, justice services, and social service organisations to support individuals leaving custody
or correctional supervision to reintegrate successfully into society.

A Collaborative Approach to Care

We actively partner with hospitals, rehabilitation facilities, mental health services, support coordinators, plan managers, and allied health professionals to ensure coordinated, holistic support.

About

Chinenye Ohiaeri

I didn’t start Zenec Home Care & Community Services just to run another service. I started it because I saw what was missing. After years of working across large organisations in the community services sector, I witnessed a pattern that didn’t sit right with me. Too often, people were treated as numbers on a funding sheet rather than individuals with real stories, real struggles, and real potential. Care became transactional. Respect became optional. And the human connection, the very foundation of this work, was lost. Zenec was built as a direct response to that. I bring over nine years of hands-on experience across homelessness services, family preservation, disability support, mental health, and counselling. My work has taken me into people’s homes, crisis situations, court-mandated environments, and complex family systems. I’ve supported individuals facing domestic violence, homelessness, trauma, and mental health challenges. I’ve worked with children at risk, young people navigating difficult systems, and families trying to stay together under pressure.

Alongside this experience, I’ve invested heavily in my education and professional development. I hold a Bachelor of Social Science (Behavioural Studies), an Advanced Diploma in Community Sector Management, a Diploma in Community Services, and I am currently completing a Master’s in Counselling. This combination of academic grounding and real-world practice allows me to approach care with both structure and empathy, evidence-informed, but always human-first. What sets me apart is how I work. I don’t believe in one-size-fits-all care. I don’t believe in ticking boxes. And I don’t believe in doing things to people instead of with them. At Zenec, every individual is seen, heard, and respected. I take the time to understand each person’s story, their goals, their environment, and their challenges. From there, I provide tailored, trauma-informed, and culturally responsive support that actually meets their needs, not just what a plan says on paper. Whether it’s complex case management, behavioural support, counselling, or helping someone rebuild independence, my focus is always the same: meaningful outcomes, dignity, and long-term impact. Zenec Home Care & Community Services is not just a provider. It’s a commitment to do this work properly, ethically, and with genuine care. Because people are not machines. And they should never be treated like one.
